If you are receiving care for a serious medical or behavioral health condition, Capital Blue Cross' care management program can help you make informed decisions and maintain or enhance your quality of life. A specially trained care manager will work with you, your family, and your doctors to help you understand your options, coordinate your treatment, and support you along the way. It's voluntary, confidential, and available at no extra cost to you.

What is a care manager?

Our care managers are highly qualified professionals who bring a wealth of experience and specialized knowledge to support our members. Here are a few details about their role:

  • Care managers include registered nurses, social workers, behavioral health specialists, health navigators, and clinical support specialists from across the country.
  • They hold certifications such as LPN, RN, CCM, and specialty certifications in obstetrics, lactation, transplant, oncology, and health equity.
  • Their combined experience includes social work, critical care, medical/surgical, home health, hospice, long-term care, maternity, oncology, transplant, dialysis, and pediatrics.
